AIN and Profile Creation Form
This survey is for potential NPTE-PT and NPTE-PTA candidates who are in need of an FSBPT account. Please fill out this survey if you attended a school outside of the United States and/or if you need an Alternative Identification Number (AIN) because you do no have a social security number. After completing the survey, please allow up to two (2) business days to receive an email with more information.
